Introduction to new project that AIRA is kicking off to support sending risk-based conditions in QBP/RSP queries to allow IIS to respond with recommendations that reflect risk conditions.
Notes
  • Risk-based forecasting (conditions not known by the IIS)
  • AIRA has received funding for draft to pilot phase
  • 12-month proof of concept project
  • Exploring Feasibility of implementation of Risk-Based IZ Forecasting
  • Looking for IIS programs, EHR vendors, IIS vendors and more to join a workgroup
  • HL7 V2 query/response method
  • Email smatlock@immregistries.org and/or follow on InteropHub to express interest

Update on PSS and socializing with Patient Care and CDS
Update on AIRA & HLN progress
Continue on Concept Review
Notes
  • Project scope statement (PSS)
  • Was just ImmDS and then we added HALO
  • Already classified as international specification
  • Need to talk to Patient Care Workgroup and need to meet with CDS Workgroup in two weeks - brining feedback back to IFG
  • HLN/AIRA Progress
    • FHIR-based exchange
    • Exposed a FHIR endpoint and consumed client
    • Reviewed request and response
    • Beta FHIR API for interacting with ICE forecaster
  • Conceptual model
    • Knowledge Base (definition agreed to last time) - an identifiable, executable set of immunization decision support rules that a CDS engine selects and applies as a single coherent unit for a forecast request. When selected, that specific set is executed and can be identified in the response is the set actually used. A CDS engine may support multiple Knowledge Bases, and each may incorporate guidance from multiple Schedule Authorities and local adaptations, but each single forecast execution uses only one Knowledge Base. Rule authorship, governance, documentation, and provenance are outside the scope of this concept.
    • Schedule Authority proposed definition - an organization or body that publishes immunization guidance intended to inform vaccination recommendations. A Schedule Authority may be associated with a federal or state government, a professional organization, an international organization, or another recognized source of guidance. This concept does not define who may serve as a Schedule Authority or assess the validity, authority, or jurisdiction of its guidance; it provides a way to identify the source whose guidance a recommendation is intended to follow.
      • Might have more than one authority within a knowledge base
      • defined broadly
      • Discussion - items that are beyond the schedule that may be relevant, might be "recommendation authority" or "recommending body" or "administration authority" or "decision authority" or "Protocol Source" or "Recommendation Source" or "Guidance source/agency", context is immunization, Is this intended to be analogous to “Immunization Schedule Used” (LOINC 59779-9)? - yes, "authority" might have a legal definition, providing information based on clinical data, interactions, funding source, shared clinical decision making
      • Decision - taking ideas back and coming back with a new short name besides "Schedule Authority", group liked the definition, but not the title
    • Different things can change independently - CDS engine software, Knowledge Base, schedule authority guidance
    • Knowledge Base Consensus
      • CDS engine may have multiple identifiable versions of KB

Discuss generally what has been decided, the current concept, and what needs to be discussed.

Notes
  • Discussed concept in the previous item. We will skip this item for now but will provide an update on this next meeting when we have a better definition for CDS Schedule Source.
