Review CDSi draft proposal for support "Vaccine Recommendation Category" and also look at HLN's approach. Start discussion on best model for this concept for use later in FHIR and HL7 v2.
Notes
  • Working Consensus
    • sufficiently agreed upon to use as the basis for continued work
    • Document and treated as settled unless new information, implementation experience or a substantive issue gives us a reason to visit it.
    • Good enough to proceed?
  • SCDM (shared clinical decision making)
    • Proposed something - want to confirm
    • Different types of recommendations - all, high-risk, or shared clinical decision making
    • Need to indicate which one
    • Recommendation Category
    • No current mechanism in HL7 v2
    • LOINC Request - still pending (been six months)
  • General support as a concept to move forward with
  • HLN Perspective
    • Category tied to schedule authority to support CDS
    • Anticipate more discussions
  • Next Steps
    • Follow up about LONIC

CDS Schedule Source Current topic
11:45 AM–11:55 AM
10 min
Start discussion on a universal system for identifying schedule source. This is the identification of expert groups at the international, country, or sub-country level that define immunization schedules for use in practice.
Notes
  • Knowledge Base
    • Defined - an identifiable, executable set of immunization decision support rules that a CDS engine selects and applies as a single coherent unit for a forecast request. When selected, that specific set is executed and can be identified in the response as the set actually used. A CDS engine may support multiple Knowledge Bases, and each may incorporate guidance from multiple Schedule Authorities and local adaptations, but each single forecast execution uses only one Knowledge Base. Rule authorship, governance, documentation, and provenance are outside the scope of this concept.
    • what's sitting in the software
    • Example - AL school immunization requirements
    • One coherent set of values
    • "Decision support knowledge base"
    • Actual vaccine rules
    • Example - "cool hep. B predictor"
    • Based upon one or more schedule authorities
  • Schedule authority
    • Who is making the language
    • Where the knowledge base came from
    • Defined - an organization or body that publishes immunization guidance intended to inform vaccination recommendations. A Schedule Authority may be associated with a federal or state government, a professional organization, an international organization, or another recognized source of guidance. This concept does not define who may serve as a Schedule Authority or assess the validity, authority, or jurisdiction of its guidance; it provides a way to identify the source whose guidance a recommendation is intended to follow.
    • Examples: ACIP, AAP
    • More discussion September 11th
